This is a reminder, per my E-Notify of August 27th, that if you do not want your child photographed or videotaped, would like to receive notification of pesticide application and / or do not want your child’s directory information released to parent / teacher organizations for student directories and news media for awards / recognitions or to military recruiters, please complete and return the appropriate form listed below by October 1, 2013:

• Pest Management / Pesticide Application Notification Form
• Student Directory Information Refusal Form
• Refusal of Release of Directory Information to Military Recruiters Form (DHHS Students Only)
